I’ve always liked SurveyMonkey.com for online survey’s. It’s simple to use and it has just enough functionality to handle just about anything you would need.
There is another tool that comes up a lot- Zoomerang. I’ve never really used it because I’ve never had any issue with SurveyMonkey.com.
